Our Mission Statement - Our aim here at Distington Surgery is to provide a quality healthcare service to the community and assist them in improving and maintaining their health through teamwork, education and specialist advice. New Patient Registration 

We are very happy to welcome patients who are new to the surgery, please be aware that to register fully you will need to provide proof of address and photo identification. We ask for this to ensure the security of your patient health record. Please contact our Practice Manager if you have any queries about this. Thank you. Due to current high demand to be registered at the practice you may be put on a waiting list and your application to join the practice will be dealt with as we have available new patient appointments

The doctors welcome new patients who live within our practice area.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the nurse and then the doctor.

Medical treatment is available from the date of registration.  Please contact reception for further information.

If you live outside of our practice area but are interested in joining the practice please complete the online registration form using the link below or alternatively collect a new patient pack from the surgery. (ID etc. required as per below)

Your request to join the practice will be reviewed by the partner GP's who will determine if it would be clinically appropriate to accept the application and you will be informed accordingly.

Patients living outside of the practice area may not be eligible for a home visit from the surgery. In such circumstances patients would be advised to contact 111 to arrange a home visit.
