Online 'Pre-Registration' With The Practice

If you wish to apply online to register as a patient at Distington Surgery please follow the instructions in full below.

1. Click on the 'Pre-registration' link below and complete all the required information, including your NHS number, full details including the postcode of your previous surgery and an up to date contact telephone number. Your NHS number can be obtained from your current/previous surgery. Once complete click send.

Please be aware that without this information your application to join the surgery will not be processed.

2. Please complete the online health questionnaire by clicking on the link below and send once complete.


3. Lastly all new patients are required to provide proof of id (passport/driving license) and proof of address (utility bill/bank statement). Please attend the surgery within 14 days of submission of all of the above with your ID to ensure your application can be processed.

Please be aware that incomplete registrations will not be processed. Unfortunately until all of the above is provided we are unable to complete the registration process. Should proof of identification fail to be provided within 14 days of submission of your application, your application to join will unfortunatley be destroyed.
